The first step in accessing CCC Plus Waiver services is to request a Long-Term Services and Supports (LTSS) screening through your local Department of Social Services. This screening, sometimes referred to as a UAI, is crucial for determining your eligibility for long-term care services.
If you qualify, you will receive a copy of your approved UAI. This assessment allows you to explore several care options, including:
Agency-Directed Care
An approved agency provides and manages a personal care aide to support your daily needs.
Consumer-Directed Care
You select the caregiver, often a trusted family member or friend, who will provide your personal care services. This model offers flexibility and control, and A&S Facilitation Advocates is here to support you every step of the way.
Nursing Facility Placement
For individuals who require a higher level of support, nursing facility care may be recommended.

Receiving an approved UAI does not automatically mean you are enrolled in the CCC Plus Waiver. Enrollment occurs only after the required documentation is completed and submitted to your insurance provider by your Service Facilitator. Services cannot be reimbursed until waiver enrollment is officially approved, making it essential to complete this process before care begins.

What Happens Next?
The next step is scheduling an intake appointment with your Service Facilitator. During this first visit, we take time to get to know you and develop a person-centered plan of care that reflects your unique needs and preferences. We will also walk through important roles, including who will serve as the Employer of Record (EOR), your Fiscal Agent, your assigned MCO Care Manager, and make sure you have a clear emergency backup plan in place to keep services running smoothly.
Once completed, your plan is submitted to your insurance provider, who determines the number of care hours approved each week. Your Service Facilitator will support you and your caregiver every step of the way, making sure all documentation is completed so services can begin without delays and caregivers are paid correctly.
The Employer of Record (EOR) plays a key role in your plan of care and is required for services. Learn more about what the EOR does and why this role is important.

We help individuals approved for CCC Plus Waiver services, including those utilizing EPSDT services, coordinate and manage their Medicaid long-term care supports through Consumer Direction.

We ensure all services are appropriate, authorized, and aligned with each individual’s goals while helping families navigate the process with clarity and confidence.
After your Service Facilitator intake appointment is completed, a referral will be submitted to your insurance provider’s Fiscal Employer Agent (FEA). The FEA plays a key role in managing employer-related responsibilities, including processing payroll, withholding and depositing taxes, verifying timesheets, and conducting required background checks for caregivers participating in CCC Plus Waiver services and Consumer Direction.
You will have the option to complete your FEA enrollment either online or using paper forms. Choosing the online option is typically faster and can help prevent delays in starting services.
